100 Club Application

Please complete the following form to register as a 100 Club Supporter. We need your bank details to pay out any winnings. They will be stored encrypted on our servers.

By completing this form, you are giving us permission to contact you by email with regards to your donation and the operation of the 100 Club. We will also email other information to you, and you can unsubscribe from that at any time.

I/we understand that payment instructions will be processed for the Teign Valley Museum and Archive Trust by Just Giving and that £10 will be taken from my/our account on the chosen day of each month until I/we advise you otherwise.

I/we agree that if we wish to cancel this payment instruction I/we will inform the Teign Valley Museum and Archive Trust of this decision as soon as possible.
